Consent to obtain and use photographs, film, visual and audio recording

Link-Up (Qld) is committed to its Quality Policy and seeks your consent to obtain and/or use photographs, film, visual and sound recordings for Link-Up (Qld) related purposes. To give your consent, please read and complete this form.

I give consent for Link-Up (Qld), its employees, assigns and agents to film, photograph and/or make visual and/or sound recordings of me as described below.

I give consent for Link-Up (Qld), to use these films, photographs and/or visual and/or sound recordings of me, including any resulting material (“the material”) in full or in part for any Link-Up (Qld), related purpose including, but not limited to, promotional, advertising and commercial activities of Link-Up (Qld) and inclusion in governmental publications. Link-Up (Qld), may use, reproduce and distribute the material in whole, in part and in conjunction with any other thing, in any manner and form which Link-Up (Qld) considers appropriate including, but not limited to print, television, radio, cable, video, Internet and any other electronic application.

Where I am the author of any work comprising or associated with the material, I give my consent under the Copyright Act 1968 section 195AW (for work included in a film) and under section 195AWA (for work not included in a film) for Link-Up (Qld) not to attribute me as the author of the work and for Link-Up (Qld) to make any amendment, deletion, alteration or selection of the work, at the entire discretion of Link-Up (Qld) and without reference to me.

I acknowledge that by giving my consent:

• Link-Up (Qld) is not obliged to use the material or exercise any of the rights granted to it by me; and
• I am not entitled to remuneration, royalties or any other payment for my participation which assists Link-Up (Qld) in obtaining the material or for Link-Up (Qld) use of the material.
• My consent and the rights granted to Link-Up (Qld) are continuous and will not be withdrawn by me.
• I have read and understood the meaning of this consent which is given genuinely and without duress.
